UBMS Athletics Schedule

• 2023-2024 Athletic Schedule

Participation

• Before participating in UBMS Athletics, students and guardians must complete the following:
  • 2023-24 Physical Form (PDF)
  • Concussion Training

Athletic Physical

• The athletic physical must be filled out and signed completely before participation in sports.
• Physicals can be obtained by your primary care physician, urgent care, or minute clinic locations.
• Physicals must be dated after May 1, 2023.
• Physical forms must be submitted to Ms. Hansen (room 1200) before school, during advisory, or before practice.

Concussion Training

• Concussion training can be completed either online or in-person.
• Information about in-person concussion training will be shared when available.
• At the end of the online training, you must print and sign the verification form. If you do not have a printer, you may download and email to [email protected]. Emails must be sent from the guardian’s email.

Admission to Athletic Events

• Adults: $3
• Students (K-12): $1
• PWCS employees, children age 4 and under, and Seniors: $0
